In the absence of screening mammography, the incidence of local and regional breast cancer rises from less than 50 per 100,000 women at age 40, to about 300 and 200 per 100,000, respectively, at around age 70, and then levels off. The incidence of in situ and distant breast cancer rises very slowly with age, never approaching 50 per 100,000.

By contrast, with annual screening at ages 50 to 74, the incidence of all stages of breast cancer spikes at age 50, drops sharply, and then rises until age 74. A sudden, sharp drop in incidence when screening ceases is followed by a gradual rise. During the period of annual screening, the incidence of in situ breast cancer is second to the incidence of local breast cancer. Before and after that period, in situ breast cancer is detected less frequently than any other stage of breast cancer.
