Notes: Quebec is excluded because cases diagnosed in Quebec from 2011 onward had not been submitted to the Canadian Cancer Registry. Net survival estimates were predicted using the period method. Caution is indicated when the standard error is > 0.05 and < 0.10; estimates associated with standard errors ≥ 0.10 were considered too unreliable to publish.
