Notes: Quebec is excluded because cases diagnosed in Quebec from 2011 onward had not been submitted to the Canadian Cancer Registry. All counts were randomly rounded to a base of five to protect confidentiality. Age-specific rates were based on rounded counts; ASIRs were not. ASIRs were calculated using weights from the 2011 Canadian standard population, see reference 26; age-specific rates were not age-standardized.
