Chart 5 Description: This is a vertical bar graph that illustrates the growth rates in the reporting of the severity of disability from 2001 to 2006.

The X axis lists the four degrees of severity, from left to right on the X axis that is, mild, moderate, severe, and very severe.

The Y axis is measured by percentage and increases by increments of five starting at 0 up to 30.

The largest growth is for mild with a rate of 26.6%.

The second largest growth is for moderate with a rate of 20.6%.

The third largest growth is for severe with a rate of 19.2%.

The smallest growth is for very severe with a rate of 16.4%.
